		Resumen
		Un profesional debe esforzarse por que el servicio que preste sea útil a sus pacientes, y al resto de los integrantes del equipo de salud. Por esto, es importante tener una estrategia a la hora de intervenir para intentar solucionar los problemas relacionados con los medicamentos, que son el centro de actuación en el seguimiento farmacoterapéutico. En la practica asistencial del farmacéutico, como en otros profesionales de la salud, a menudo no se consiguen resultados sin el apoyo del paciente y del médico. Es preciso aplicar una base científica, pero también conocer que sistema de valores y deseos tienen pacientes y médicos. En este artículo se presenta un modelo de estrategia de Intervención, que ayudará a decidir sobre que PRM se debe actuar y como. Se basa en evaluar en nuestro paciente y para cada PRM la ecuación valor/esfuerzo, analizando factores que aportan valor y en que medida y cuales aportan esfuerzo. Nuestra actuación a la hora de intentar solucionar un PRM estará en maximizar valor y minimizar esfuerzos. De esta manera se les proporcionará a los pacientes los servicios que necesitan de la manera que ellos desean. Se ha de conseguir que los pacientes deseen cubrir sus necesidades relacionadas con los medicamentos.

		A professional must encourage for the provided service to be useful to his or her patients and to the rest of the members of the health care team. So, it is important to have an strategy on intervening on trying to solve pharmacotherapy failures, which are the focus in pharmacotherapy follow-up. On pharmacist caring practice, the same than other healthcare professionals, often positive outcomes are not achieved without patient and physicians support. It is necessary to apply a scientific background, but it is also necessary to know what values and desires patients and physicians have. This paper presents a model of intervention strategy , that will help to decide which pharmacotherapy failure intervene on and how. It is based on evaluating in each patient en in each pharmacotherapy failure, the ratio value/effort, by analyzing issues providing value and how much, and which produce effort and how much. Our way of acting on trying to solve pharmacotherapy failure should enhance value and reduce efforts. In this way, our patients will be provided by the services they need, in the way they want. Our goal will be to achieve our patients desire to face their pharmacotherapy needs.
